What is usually behind back and neck pain
Spinal joints are built to move. When one stops moving properly — after a long stretch at a desk, a bad night’s sleep, lifting something awkwardly, or for no reason you can name — the surrounding muscles tighten to protect it. That guarding is what most people actually feel: a deep ache, a stiff neck that will not turn, a back that seizes when you stand up.
The joint and the muscle keep each other stuck. Treating only one of them is why pain often comes back a few days later, and why working on the sore muscle alone can feel wonderful on Tuesday and irrelevant by Thursday.
Most of this responds quickly once both are addressed. Some of it does not, and that is worth knowing early too — if your pain is not behaving like a mechanical problem, you should hear that from someone rather than keep booking visits.
When to go to urgent care or the ER first
Some back and neck pain is not a chiropractic problem. Go to urgent care or an emergency room promptly if the pain comes with any of these:
- Numbness in the groin or inner thighs

- Weakness in a leg or arm that is getting worse
- A fever alongside the pain
- Weight loss you cannot explain

No. There is no contract, no prepaid package and no standing monthly visit. Dr. Penner gives you a visit estimate after the exam, you pay for each visit you have, and when you are done, he says so. If your back flares up again next year, you book again then.

Is a chiropractor worth seeing for back pain?
Often, yes, when the pain behaves like a mechanical problem: stiffness, a deep ache, a back or neck that locks up with certain movements. In its 2017 guideline, the American College of Physicians lists spinal manipulation among several non-drug options for low back pain, while rating that evidence as low quality (NIH summary). It is a reasonable option, not a miracle, and by the third visit there should be a clear direction either way.

Can a chiropractor bill insurance for massage?
Yes. In Washington, massage and trigger point therapy are part of chiropractic care under state law (RCW 18.25.005), and chiropractors may bill for the services the law allows (WAC 246-808-540). At SoundCare, Dr. Penner does the massage himself, alongside the adjustment in the same visit, and it is billed with that visit to your health insurance or, after a car accident, your PIP.
