Consumer Health Data Privacy Policy
The disclosures RCW 19.373.020 requires, and how to exercise your rights.

This is the consumer health data privacy policy of SoundCare Chiropractic, published under the Washington My Health My Data Act (MHMDA), RCW 19.373.020. It applies to Washington residents and to anyone whose consumer health data we collect in Washington.
Most health information SoundCare holds is protected health information governed by HIPAA, which the Act does not reach. This policy covers the narrower set of data our website collects outside HIPAA: what you tell us before you become a patient, and health-related inferences drawn from how you browse our service pages.
Categories of consumer health data we collect, and why
Conditions and symptoms you describe
For example: neck pain since a collision, chronic lower back stiffness, whiplash.
Purpose: to respond to your inquiry, determine whether we can help, and schedule appropriate care.
Health care services sought
For example: requests for auto injury care, general chiropractic care, or spinal rehabilitation.
Purpose: to route your request and prepare for your visit.
Bodily functions you volunteer
For example: range-of-motion limits, headaches, sleep disruption.
Purpose: to assess urgency and prepare for your visit.
Signals that you are seeking care
For example: the fact that you submitted a form or booked an appointment on a chiropractic website, and the pages you viewed.
Purpose: to provide the service you requested and to measure site performance in aggregate.
Precise location
We do not collect precise location data.
Categories of sources we collect it from
You, directly, through our contact and new-patient forms, our online scheduler, email, and phone.
Your device and browser, automatically, through cookies and website analytics.
We do not purchase consumer health data and do not obtain it from data brokers or from any third party other than the processors listed below.
Categories of consumer health data we share
Appointment details you submit through the scheduler, shared with our scheduling vendor to complete your booking.
Contact form contents, which pass through our website host and email provider in the course of delivery to us.
Pseudonymous browsing data, including the fact that a visitor viewed a particular service page, processed by our analytics provider.
We do not share any other category of consumer health data.
Third parties and affiliates we share it with
We have no affiliates. The specific third parties are:
- Jane App, appointment scheduling processor
- xCloud, hosting and server logs
- Brevo, delivery of contact form submissions
- Google LLC, Google Analytics
- Jane.app, once information becomes part of a patient record
We do not sell consumer health data
We have never sold consumer health data and do not offer it for sale. The Act makes it unlawful to sell consumer health data without a signed, separate authorization, and we do not seek one because we do not engage in the practice.
Your rights and how to exercise them
Under RCW 19.373.040, you have three rights. Exercising them is free and will never cause us to deny you care, charge you a different price, or provide you a lesser level of service.

Confirm and access
Confirm whether we are collecting, sharing, or selling your consumer health data, and access that data. That includes a list of every third party and affiliate we have shared it with, plus an active email address or other online contact for each of them.

Withdraw consent
Withdraw your consent to our collection and sharing of your consumer health data at any time.

Delete
On request we will delete your consumer health data from our records, including archived and backup systems, and notify the processors and third parties listed above, who are required to honor the request.
To make a request, email privacy@woodinvillechiropractor.com with the subject line Washington Health Data Request, write to us at 17924 140th Ave NE, Suite 100, Woodinville, WA 98072, or call 425-215-1880. Tell us which right you want to exercise and give us enough information to locate your records. We may need to verify your identity before acting.
We will respond within 45 days. If we need more time, we will tell you within that first 45 days and may take up to 45 additional days.
If we deny your request, we will explain why and tell you how to appeal. We will decide your appeal within 45 days. If we deny the appeal, we will give you a method for contacting the Washington State Attorney General to submit a complaint.
Deleting website data does not delete your medical record. Washington law and our professional obligations require us to retain patient records for a set period. Requests about your medical record are handled under HIPAA and our Notice of Privacy Practices.

You may file a complaint with the Washington State Attorney General’s Office at atg.wa.gov/file-complaint or by calling 1-800-551-4636.
Changes to this policy
If we begin collecting or sharing categories of consumer health data, or using it for purposes, not described above, we will disclose the change and obtain your affirmative consent before doing so, as the Act requires.
